Private sessions run anywhere from about $90 to $300, but very few people actually pay the sticker price. Extended health benefits, the HST exemption that landed in 2024, supervised intern programs and sliding-scale spots each chip away at that number. Stack two or three of them and your real out-of-pocket cost can drop to almost nothing.

Cost is the reason a lot of people put off booking. So here’s the honest version of how affordable therapy in Ontario actually works, minus the jargon.

Does your extended health insurance cover therapy?

Often, yes. If you have a workplace or private plan, mental health coverage is usually sitting there under “paramedical” services. Plans tend to name the exact professional they’ll reimburse, most commonly a Registered Psychologist or a Registered Psychotherapist in Ontario. What varies wildly is the dollar amount. Some plans cap out at a few hundred dollars a year. Others cover a few thousand.

Two questions settle it before you book. What’s my annual maximum for psychology and psychotherapy? And which designations are covered? Ask your insurer both. Even clinics that don’t bill insurers directly (ours included) hand you a detailed invoice to submit for reimbursement, so direct billing is only a convenience, not a dealbreaker.

Do you still pay HST on therapy in Ontario?

No, not anymore. As of June 20, 2024, psychotherapy services are exempt from GST/HST across Canada. Psychologists’ services were already exempt before that. In plain terms, that’s 13% gone from the top of every session fee in Ontario. Over a full course of therapy, that’s real money back in your pocket.

Can supervised intern sessions lower the cost?

This is the option people miss most. Plenty of clinics run reduced-fee sessions with master’s-level psychology and psychotherapy interns, who work under the close supervision of a Registered Psychologist or Psychotherapist. You still get trained, evidence-based care. You just pay less for it. At Therapy Villa Psychology Services, supervised sessions are $90 for 50 minutes, and they’re frequently still claimable under extended health plans, which stretches your coverage further.

Do therapists offer sliding-scale fees?

Some do, and not always publicly. A number of therapists hold back a few reduced-fee spots for clients who need them, based on financial circumstances. The catch is that these spaces are rarely advertised. So ask. A direct question during a consultation costs nothing and occasionally opens a door you didn’t know was there.

Is online therapy cheaper than in-person?

In most cases, the session fee is identical. The real-world cost usually isn’t. Online sessions cut out commuting, transit or parking, and the childcare you’d otherwise need to leave the house. They also widen the field. You’re no longer limited to therapists within driving distance, which means you can look for lower-rate practitioners anywhere in Ontario instead of just in your own neighborhood.

Does OHIP cover therapy in Ontario?

Not for private practice. OHIP does not cover sessions with a private psychologist or psychotherapist. It does cover some mental health care delivered by physicians, including family doctors who do psychotherapy, plus care through hospital and community programs. The trade-off is the wait, which can be long. Community health centers, university training clinics, and non-profits are worth a look too, since many run free or low-cost support.

Putting it together

For most people math works like this. Lean on your insurance first. Let the HST exemption do its thing automatically. Then ask about supervised-intern or sliding-scale rates to cover whatever gap is left. You almost never have to choose between your mental health and your budget. There’s usually a combination that gets you in the room. If therapy is essential to your well-being, it’s worth budgeting for it as part of your monthly expenses.

Frequently asked questions

How much does therapy cost in Ontario without insurance?
Most private sessions land between roughly $90 and $300. Supervised-intern and reduced-fee sessions sit at the lower end, around $90, while a session with a Registered Psychologist sits at the higher end. Since June 2024, there’s no 13% HST added on top.

Does insurance cover couples therapy in Ontario?
It depends on your plan and the therapist’s designation. Many plans reimburse sessions with a Registered Psychotherapist or Psychologist, and couples sessions are often eligible. Confirm your annual maximum and covered designations with your insurer first.

Is virtual therapy covered the same as in-person?
Usually, yes. Most plans, including EAPs, reimburse virtual and in-person sessions at the same rate. Virtual sessions generally require you to be located in Ontario at the time of your appointment.

What’s the cheapest way to get therapy in Ontario?
A supervised-intern program or a sliding-scale spot is typically the lowest-paid option, and either can be stacked with your insurance coverage. For no-cost support, look to OHIP-covered physician care, community health centers, and university training clinics, though waitlists apply.

Therapy in Ontario costs less than most people think, once you know where to look.
